Flexible, round-the-clock services are provided to individuals with a mental health diagnosis, centered around drop-in visits at home or on-site. Offered on a drop-in basis in the person’s home, an on-site facility, or in a setting most conducive to promoting the participation of the individual in community life. Clients receive at least 6 visits per month from a Rehabilitation Counselor to enhance skills, independence, and access to community resources.
The Targeted Case Management Program provides case management services for adults in Calvert, Charles and St. Mary’s counties. TCM ensures that persons with mental health needs have the resources they need to enhance their ability to function independently in their community. TCM provides connections to resources and support to meet their basic needs.
A 15-month program helps adolescents (ages 5-17) with mental health issues at risk of foster care or residential placement. It assists those with frequent inpatient, partial hospitalization, or ER visits, or involved with DJS, DSS, or facing school suspensions with an IEP/504 Plan. The IHIP Case Specialist works with the child, family, school, and mental health providers to improve functioning and home environment.
Youth PRP is offered as a step-down program from IHIP-C or if a child has less intensive needs. The PRP offers services to children between the ages of 5 and 17 years old with a mental health diagnosis. These children must be involved in outpatient mental health services to be eligible for the program and must maintain this involvement throughout services.
The Porter House Residential Crisis Program offers 24/7 assessment and stabilization to individuals at risk for hospitalization or transitioning from hospitals, aiming for short-term stabilization. It caters to acute crises in emotional, mental health, and/or substance use, providing a person-centered approach and a dignified environment.
Assertive Community Treatment is an intensive approach to behavioral healthcare in the community. It features an interdisciplinary team that work together to serve individuals who have struggled to benefit from traditional outpatient services. The program aims to serve people who are dealing with homelessness, frequent use of hospitals, or those being released from hospitalization.
The Residential Rehabilitation Program offers transitional group homes and supportive psychiatric rehab services to individuals with severe and persistent mental health diagnoses while they learn to manage their mental health, improve their daily living skills, and meet their personal goals.
Our Supported Employment/Vocational Program reintegrates individuals with mental health diagnoses into meaningful employment in their community. Employment Specialists assist with goal assessment, job skills evaluation, applications, job searches, interviewing, communication, resume building, job coaching, and placement. Disability benefit recipients receive counseling to maintain benefits while working.
