Integrated Health
Cornerstone Montgomery’s Integrated Health Program is part of a National and State effort to integrate somatic and behavioral health services in Maryland.
Our Integrated Health program focuses on individuals with behavioral health needs who are at high risk for additional chronic conditions such as diabetes, obesity, and heart disease.
Participants receive enhanced care coordination services from providers with whom they regularly receive care. Services are offered through our Psychiatric and Residential Rehabilitation programs and Health Homes staff. These services are open to all participants in our Residential and Psychiatric Rehabilitation Programs.
